CONSENT WAIVER & RELEASE
1st Annual Gold Shield Foundation Ride" Date of Event: November Xth,
2009
I agree to abide by all Federal, State, and Local laws
and regulations and Event Sponsor's policies and procedures for the "
Gold Shield Foundation " motorcycle ride (hereinafter referred to as the
"Ride) for my personal safety, for the safety of others, and as
applicable, the operation of a vehicle during the Ride. I will take all
safety precautions, including my use and the use by my passengers I may
have of safety equipment and attire, including without limitations,
helmets, appropriate clothing and eyewear. I warrant and represent that
I have current liability insurance for the operation of my vehicle and
have provided proof of same.
If I will participate in the Ride as a passenger, I agree to all terms
and conditions of this Consent Waiver and Release as they may be applied
to my participation in the Ride as a passenger.
In Consideration of the Event Sponsors permitting me or as applicable
any minor under my guardianship, to participate in the Ride (
hereinafter referred to as "Participant"), on behalf of myself, my
heirs, executors, administrators, successors and assigns, Participant
does hereby indemnify, release and forever discharge any claims,
actions, demands, costs, expenses and damages of any kind or character
that Participant may have against the Event Sponsors, Hard Rock Cafe,
Atlanta Harley Davidson, the Gold Shield Foundation and any chapter
thereof, or any third parties, in connection with the municipalities or
property through or over which the Ride will take place, as well as any
other persons or entities connected with the Ride, together with their
heirs, executors, administrators, successors and assigns for any
injuries or damages that may result from Participants involvement with
the Ride in any manner whatsoever.
All proceeds go to Gold Shield
Foundation of Metro Atlanta, a 501(c)(3) providing direct financial support for Police
Officers, Firefighters and their families who are injured or killed in
the line of duty.
